Wayfaring MD

I am a family medicine resident who likes to highlight the hilarious in medicine as I write about patients, medical school, residency, medical missions, and whatever else strikes my fancy.

HIPAA is for reals, folks. All of my "patient stories" have been changed to protect patient privacy. I will change any or all identifiers, including age, location, race/ethnicity, sex, medical history, and quotes.

Onchocerciasis: River Blindness

Onchocerciasis, the disease transmitted by the bite of the black fly, is fairly common here. We commonly have patients asking to have various lumps and bumps removed. Sometimes they’re lipomas; other times they’re cysts; then other times they’re oncho nodules. The oncho worms wall themselves off in little nodules so they can replicate without danger of being found by the host’s immune system. After treatment with ivermectin, the worms die and the nodules become calcified.

Above Left: removing an oncho nodule on the iliac crest. Oncho nodules are often found over bony prominences.

Above right: split open oncho nodule with calcified worm in the center.

**Disclaimer: I purposely do not disclose the name of the country I went to in Africa. I did the same when I went to South America. I keep the specifics secret for the sake of my and my patients’ anonymity. There are not tons of mission hospitals or family medicine residents on the continent, so giving specifics away would make it too easy to potentially identify me.
